Another excellent installment on the practice of identifying and capturing lessons-learned knowledge by Dennis Pearce.

I would add to this article that company managers make "trade-off" decisions all the time. Trade-off decisions are lessons-learned because the rational process to make trade-off decisions forces people to drill-down to understand the conditions that affect those decisions (available resources; market conditions; available personnel, competing initiatives, etc.).
The on-going problem that KM practitioners have is a means to record these valuable nuggets of knowledge, and sharing them with those who need to know.
